Download The Forms — There are three basic forms you need to bring to your first visit. Downloading them here and completing them before your appointment saves time on your initial visit.
- Form 1: Patient Information Form;
- Form 2: Medical Intake Form;
- Form 3: Privacy Notice & Agreement
Prepare For Your First Appointment — Remember to bring the completed forms, correct clothing to change into and a lock if you wish to use our lockers. Please arrive about 10 minutes prior to your appointment time to hand in forms and allow time to change. If you are not bringing completed forms, then allow 20 minutes prior to your scheduled time.
